The Utah Family Voices and USU Center for Persons with Disabilities presents Health Transitions: Top Five Things To Know on Tuesday, July 11 at 6:30 pm to 8:30 pm at Active Re-Entry located at 10 South Fairgrounds Road. This event is open to all youth and young adults with special healthcare needs and disabilities preparing for a healthy transition into adult services.

This open house will feature an interactive session about transitioning into adult health care and learn about related services, support and resources that are available, plus attendees can meet other individuals that are going through the same experience.

There will be light dinner served for guests to enjoy. There will not be child care available at the event so please make prior arrangements.
