Please upload your insurance card when completing the form. If you don’t pre-register, you are required to show your insurance card or pay with cash. If available, bring your Immunization card or record with you.

Cash Pay: $35 Regular Flu Shot | $85 High-Dose
*FREE* Flu Shots for the first 300 Utah State Eastern Student/Staff/Faculty (must provide *A-number).

The 2025 Utah State Eastern Flu Shot Clinic is hosted in conjunction with the Southeast Utah Health Department and USU Nursing. Thank you for supporting your future nurses and our local health department in this effort and protecting each other!
