Active Re-entry is a community-based program which assists individuals with disabilities to achieve or maintain self-sufficient and productive lives in their own communities and home.

“We have a large geographical area that we cover and I think most people would be surprised to learn that Active Re-entry covers seven counties. All Carbon, Emery, Grand, San Juan, Duchesne, Daggett and Uintah Counties. We have three satellite offices to help maintain that coverage, one is in Montezuma Creek, the other is in Moab, and our third satellite office is in Vernal,” said Director, Terri Yelonek.

Because of the COVID-19 pandemic, the offices have had to suspend some of their services to accommodate the social distancing directive. “I have a wonderful team that I am blessed to be able to work with each day and they’ve been very creative to figure out how we can continue to maintain services to the consumers that we provide service to. But what they’ve been able to come up with is we are calling our consumers on a regular basis to check in with them to make that their needs are being met,” said Yelonek. Active Re-entry is doing their best to stay in constant contact with their clients during this difficult time. They are even offering youth and adult groups via online video or by conference calls

“In addition, we’re still able to provide loan-bank equipment which is durable medical equipment that people may need on a loan and if you call the office we have two members who are able to arrange giving you that equipment that you might need and do it in a safe way,” said Yelonek. The Assistive Technology and Equipment Loan Bank program is designed to facilitate consumers in obtaining necessary assistive technology.
